Botox ® The Facts

Botox® is the trade name for Botulinum Toxin which is a prescription drug, and therefore it is unlawful for anyone other than a qualified Medical Practitioner to prescribe. Despite its popularity, Botox® is not a treatment to be taken lightly, with possible side effects if administered incorrectly, or used on those with certain medical conditions.

Botulinium Therapy

There are many factors that contribute to the ageing of our skin (sun-damage, alcohol, smoking, etc), lines develop because the muscles underneath cause it to crease when we use them to create facial expressions. Botox® relaxes specific muscles allowing the overlying skin to smooth out. The principle facial areas treated are central frown lines, forehead creases and wrinkles around the eyes.

Botox® is given by injection directly into the muscles concerned. An anaesthetic cream is applied to the skin before hand, which numbs the area being treated. Any discomfort is consequently very mild and of a temporary nature.

Botox® has been researched since the 50’s and has been used medically for many years to treat muscular disorders. It can be used anywhere in the body to relieve the effects of abnormal muscular contractions.

The dose for cosmetic use is very small and side effects are rare and will be discussed fully at your initial consultation. An unexpected benefit from the cosmetic use of Botox® has been reports of relief from tension and migraine headaches.

In most people the effects become apparent after 7 to10 days and initial results last an average of 3 to 6 months. The muscles then gradually regain their strength. Clinical trials report that patients who have undergone three therapy sessions, only require further treatment once or twice a year. This is probably due to the fact that the muscles weaken through inactivity and the frowning habit is lost.

Afterwards you may be a little red or swollen at the treatment sites. This usually resolves within a few hours, you may feel some tenderness for a few days – rarely there can be bruising. There are a few things you must be aware of e.g. you must not massage the area for a period of 24 hours. An aftercare instruction sheet will be given to you following your treatment with further advice.

Filler Treatments


Restylane® and Juvéderm® Treatments

style=Fine lines and wrinkles can now be successfully improved with modern fillers, which consist of Hyaluronic acid, a substance found naturally in the body.

As we age its volume diminishes in the skin, leaving wrinkles and other signs of ageing.  Hyaluronic acid is formulated to be injected into the skin to lift up the tissue of a line, wrinkle or a fold.

Recent research has shown that the injection of Hyaluronic acid also stimulates the body’s own fibroblast cells to produce more natural collagen, which makes the skin smoother and more elastic.

There is a whole range of filler products that have been developed for use in different areas of the face and lips. As they are not extracted from animals, they are extrememl;y safe. It is not necessary to perform pre-testing.

During treatment there may be some mild temporary discomfort, but this can be alleviated by the use of local anaesthetic.

As with any treatment that requires injection, patients may experience some redness or swelling, this is normal and will generally disappear in a day or two.  Some people may experience a little bruising. Again this will be temporary and can be covered with light make-up.

The majority of patients will only need further treatment after approximately one year.

In my experience the two best and safest filler materials are Restylane® manufactured by Q-Med and Juvéderm® manufactured by Allergan (who also produce Botox®).

They both have a good range of light to heavy materials depending on where they need to be used. Most Restylane® and all Juvéderm® products contain Lidocaine which is a local anaesthetic and this reduces considerably any discomfort during the procedure and also for several hours afterwards.

These two great ranges of fillers enable me to choose the absolute best product for the procedure in question, knowing they have a tremendous safety record.

Facial Rejuvenation Treatment

Restylane® Vital

‘Vital’ is made from the same non-animal stabilised Hyaluronic acid as the other filler products. However, unlike these, it does not contain particles but has been developed as a completely smooth gel.

In this form it is not used as a filler, but is designed to boost the skin’s natural hydration and so counter the effects of ageing and sun damage.

Many tiny applications of Vital are used over the area to be improved and the effects are stimulatory on the body’s own collagen producing cells.

It not only moisturises the skin from within but also stimulates the cells that produce natural collagen (the fibroblasts), this results in more elastic & smoother skin.

During treatment surface anaesthetic cream will be used to ensure that any discomfort is kept to an absolute minimum.

As with any treatment that requires injection, patients may experience some redness or swelling, this is normal and will generally disappear in a day or two.  Some people may experience a little bruising. Again this will be temporary and can be covered with light make-up.

Initially, the areas to be re-vitalised will be treated once a month for 3 months, following this a maintenance session will be necessary approximately every 6/12 months.

FUTURE MAINTENANCE AND FREQUENCY OF TREATMENTS





For maximum effect, it is important that a muscle relaxation treatment such a Botulinum Toxin is repeated on a regular basis.

Continuous muscle contractions will cause progressively deepening skin creases and with the additional effects of ageing, which causes the skin to become thinner, the creases gradually become more permanent.

Botox® relaxes the muscles and as a result of this, the skin gradually regains its elasticity and the creases become less  noticeable.

If the procedure is repeated every 5/6 months, the muscles will progressively weaken and the underlying ligaments will strengthen – this results in a further gradual improvement over several treatments which can then be maintained.

If too much time is left between sessions, the skin will again be creased and the muscles strengthen, thus losing the benefit of ongoing improvement.

It is in everyone’s interest to maintain therapy at regular intervals to maintain the ongoing benefit.

Likewise, when filler is used there is a secondary effect of cellular stimulation producing natural collagen within the treated area – this again results in increased elasticity and skin thickness. Most filler treatment will last approx. one year, but after this the effects can ‘fall away’ quickly.

In order to encourage regular treatment and thus to maximise the effects, I will offer a 10% reduction to all Botox® treatments carried out within 6 months of the original session and will retain this discount permanently for all patients who continue to receive therapy successively within the following 6 monthly periods.

Also, 10% discount will be applied to all filler treatments carried out within 18 months of the first session.

Please contact reception, if possible, several weeks before an appointment is needed in order to ensure a booking is available – alternatively, appointments can be  pre-booked ahead at each session if this is easier.

Obagi® Rejuvenation and Skincare Regime


Obagi Nu-Derm® System is a physician dispensed, prescription strength skincare system that can actually transform your skin at the cellular level.

What can you expect from Nu-Derm System?

In order for the Nu-Derm System to correct the function and health of your skin, it uses prescription-strength ingredients to penetrate to the deepest layers and affect your skin at cellular level.

During the four distinct phases of your skin transformation, daily use of ingredients in the system will restore cellular turnover, providing noticeable results in as few as four weeks. While friends and family will notice the difference in your skin early on, continued use of the system will reveal clearer, firmer, more even toned skin over the course of just 12 to 18 weeks. It’s this complete transformation process that enables your skin to look and act younger and healthier.

How does the system work?

Treatment Phase – 3/4 months

Existing Old Skin – The outer epidermis becomes thicker with dead old cells and too much keratin (scale) so the surface is rough and nodular (bumpy) – the keratinocytes that produce keratin have become abnormal and not functioning properly.

Nu-Derm causes the basal (deep) layers of the dermis to produce more young normal keratinocytes – so that the outer corneum becomes thinner and smoother like it used to be.

Existing Old Skin - Has areas of pigmentation (dark spots): This is caused by too much melanin because the melanocytes become abnormal and over productive.

Nu-Derm causes fresh cells to be formed resulting in normal skin tone and reduction of pigmentation.

Obagi® Nu-Derm System

Daily Routine

Morning

Prepare:

a) Cleanse skin – using Cleanser or Foaming Gel wash and rinse with lukewarm water and pat dry.

b) Toner* – apply with cotton wool ball or pad to whole face – do not rinse.

*Toner alters the Ph of the skin to make it receptive to active substances.

Correct:

Clear: Apply ½ gm evenly over the entire face to hairline. [After 2 mins can apply a 2nd coat to dark areas / spots]

Exfoderm or Exfoderm Forte: Apply ½ gm over entire face – avoid eyelid area.

All creams should be rubbed in gently until absorbed before proceeding

Protect:

Sun block if summer. Eye cream to use around the eyes and eye lids – i.e. within and around the orbit.

Evening

Prepare:

As morning

Correct:

Clear only ½ gm

Stimulate:

½ gm Blender

½ gm Tretinoin

Mix together and rub into full face avoid eyes.
